Integra Healthcare Ltd is seeking two highly experienced Paediatricians (and/or neonatologists), to join our very busy practice here on the lovely island of Grand Cayman in the Western Caribbean. You’d be working alongside our existing, significant team of paediatricians, including 2 UK tertiary neonatologists, with additional colleagues in family medicine, women’s health, dermatology, ENT and more, as well as our wider group physicians & partners, providing access to extensive diagnostic services, both imaging and laboratory, along with hospital-based services, wider specialists & surgeons, and allied professionals e.g. nutrition.

The Role & Primary Requirements

We are looking for a number of experienced paediatricians or neonataologists who are seeking the right balance between a stimulating, busy work life, a very favorable level of remuneration and great work-life balance. Crucial to us is finding the right team members, with the right attitude, so if the team around you is just as important to you, we think we might get on just fine. Whereas we have a degree of flexibility over professional credentials, there are a few things that are important:

Paediatrics

Integra has the largest private general & newborn paediatric practice in the Cayman Islands, and you’d be joining 4 colleagues with a diverse range of skills and interests. Consequently, we have some flexibility in the skills and capabilities we’d like but we are looking for at least one with general paediatrics as a primary focus and one with neonatal medicine as the focus. Additional to these primary areas, someone with special interests would work particularly well within our multispecialty practice.


Special Interests

If you have a special interest, we are extremely keen to hear about it, given our multi-specialty practice and embracing of a multidisciplinary approach. Areas that would suit our practice including (not in order!):

  • Adolescent Health Needs
  • Neurological Problems
  • Endocrinology

However, we can accommodate a variety of interests and would be only too happy to discuss this with you.

Role Complexity & Nature of Work

We are very keen to find someone who has a broad range of capabilities across paediatrics and/ or neonatal medicine, given the size of our population. Secondary and tertiary referral is sometimes complex due to Island-status but we are often the go-to practice for more complex care due to the calibre of individuals we have and our access to the most comprehensive range of secondary or tertiary, diagnostic and hospital-based services on Island. Consequently, the competencies and special interests you have will obviously have a bearing on the complexity of your work but in any event, as an island, good clinicians tend to attract interesting work.

 

A Perspective from Dr Sara Watkin, Co-founder & Consultant Paediatrician & Neonatologist

I am a tertiary neonatologist, coming from one of the UK’s premier services that accepted a very complex caseload. However, here, I provide extensive primary care paediatrics, as well as both secondary and tertiary paediatrics and neonatal care for private patients, and also act as one of the go-to neonatologists for the Government too, when it comes to complex or very preterm infants. Consequently, I have exceptional variety and definitely enough complexity. I have also loved returning to fully hands-on clinical medicine. The variety is wonderful.

 

BACKGROUND

In terms of this appointment, we aren’t unduly concerned whether someone has come from a 1-2 person independent practice or a more substantial group one. What’s more important is a breadth of capability, rather being too niche or sub-specialist. A special interest is not necessary but if you have one, you’ll undoubtedly find yourself drawing in these cases, alongside the more bread and butter ones.

Because the Cayman Islands is not especially a training environment, you need to be hands on and comfortable with doing ‘all’ your specialty entails, without delegating to juniors. That’s more of an adjustment for an experienced tertiary specialist than a GP!

  • 3 years post-CCT experience, which is a licensing criteria for Cayman Islands

Remuneration, Terms & Conditions

Obviously this will be influenced by experience and skill sets but we can definitely set some parameters that will help you evaluate the opportunity. We are somewhat unique on the island by remunerating our doctors with a robust salary to start, rather than a limited one. Additionally, we further remunerate based on a share of revenue (which is completely standard on the island) but more uniquely we do so on billing, not on collection like the majority, which removes the doubt and impact of any poor collection practices (we are very efficient!). As a starting point, the following is a reliable guide:

  • Basic salary of not less than $120,000 KYD per annum (equates to £120,000 GBP) and likely around $150,000 KYD depending on experience
  • Additional billing-related annual earnings after period of getting established (we can elaborate to the interested)
  • Full indemnity cover (robust Lloyds cover)
  • All work permit fees (medical work permits are $13,650 per annum)
  • Health insurance (Choice of plans)
  • Pension as required by law (do not get excited by this but see below)
  • Holiday and Study Leave

Cayman Islands are a tax free jurisdiction. Consequently, for professional grades of staff, saving into a pension is no more tax-beneficial than any other savings or investment strategy. However, a pension is a requirement of the law at the level of 5% employee and 5% employer contributions, up to a maximum annual salary of $87,000 per annum.

We are aware that remuneration is SIGNIFICANT compared to a UK paediatrician, even before the tax free status is considered. However, we want to be abundantly clear that we are looking for individuals motivated by what the work and life represent and who wish, like us, to work hard and consequently enjoy a robust financial return alongside a very nice work-life balance. If we get any sense someone is coming ‘just’ for the money, that’s a screen out for us. However, we are absolutely certain that remuneration will not be a barrier to anybody!
